Please review the forms and rules below. All teams are required to submit a signed Team Responsibilities Form when completing your online check in. All players on your roster must have valid medical release forms (you can download ours, our use your own club's form) that you must bring with you to the tournament. If a player would not like their picture to be used on our website or social media pages, please send a completed Photo opt Out Form to the Tournament Director.
